Complete the information in the KUMHO REBATE form, attach proof of purchase (invoice), and mail to the address listed in step 3 on the form. Valid only on a set of four (4) tires listed above at participating GB Auto locations. Rebate form must be postmarked within 30 days of purchase. Please allow 4 – 6 weeks for Card delivery. Submission and postmark deadline is 1/31/2021.
